Description	Use this form to change a student's curriculum or request a new advisor. If changing curriculum the form goes to Registrar's Office, then it will go to Tobi Hay for an advisor change, if appropriate. If only an advisor change is being requested, it can be sent to Tobi directly. Curriculum changes will not be made after the start of a semester due to the risk of interrupting a student's financial aid.
