Book Purchase Request Form

Please use this form to request a book for the library's collections. Other (non-library) book requests should be forwarded to the Business Office. Purchase suggestions for the library's media collections can be submitted using the link on the library's media page. The Douglas Library Collection Development Policies are available for your review on the library on the library home page. We will notify you with the status of your request.
