Interlibrary Loan Periodical Request Form

Please fill out a separate form for each item you wish to request. You will be notified by email when the item you requested has arrived. The Douglas Library does not charge a fee for processing your Interlibrary Loan request. However, some libraries do charge a service fee for providing their materials.
