Student Health Records and Requirements


New York State Public Health Laws 2165 and 2167 require that all students attending college and universities who were born on or after January 1, 1957 and registered for 6 or more credit hours to submit documentation of immunity to measles, mumps and rubella (MMR).    

In addition, All students, regardless of age or number of credits, must complete the Meningitis Response Form.  Please note that it is NOT mandatory to get the meningitis shot, but NYS Law requires that each student complete this form and submit it to the Health Office.  The NYS Public Health Law also states that a student will not be allowed to remain enrolled should they fail to submit these records.

Services Provided:

  • First-aid treatment of minor accidents and illnesses.
  • Basic assessment of ill students and make referrals to local medical and dental healthcare providers.
  • Provide health-related information.
  • Assess student records for compliance with NYS Public Health Laws 2165 and 2167 regarding immunizations.
  • Provide health clearance for all first and second year nursing students.

The Health Office is open during the Fall and Spring semester on days when classes are in session from 10:00 a.m. - 2:00 p.m.
