Nursing Student Health Requirements

To protect the health of all Clinton Community College members and the patients that you serve, CCC and our clinical partners require students to fulfill certain health requirements.

Nursing students are expected to submit their health records report to Health Services at least two weeks prior to the start of classes. Associated fees will be the responsibility of the student.

The vaccines listed are to be reported by your physician on the Immunization Record contained in the Health Report. A physical form is also included in this packet.

Please use the forms that are included in the packet to ensure ALL required information is submitted.
