Employee Network Account Request Form
After receiving confirmation from the college, each new employee must personally complete the form to ensure they understand and will abide by the college's Acceptable Use Policy. Every effort is made to create the account within 2 business days.
Your username is created by using the first four letters of your last name, the first initial in your First Name, and the first initial in your middle name. For example, if my name was Oliver Wendell Holmes, my username would be holmow. For your first login use the Temporary Password you supplied then change your password when prompted.
Multi-Factor Authentication (MFA) will be required. Multi-factor Authentication (MFA) is an authentication method that requires the user to provide two or more verification factors to gain access to a resource such as an application or online account.
Your supervisor will be notified once the account has been created.
